The referral rules the scorer applies

The Checklist guidance requires a full assessment with: two or more domains selected in column A, five or more in column B, one A and four Bs, or one A in a domain that carries a priority level (Checklist guidance, paragraph 19). Our guide to the CHC Checklist explains all 11 domains, and the eligibility criteria guide covers the full test that follows a referral.

Common questions

What CHC Checklist score do you need?

A full assessment is required with two or more domains at level A, five or more at level B, one A plus four Bs, or a single A in one of the four priority domains: Breathing, Behaviour, Drug therapies and medication, and Altered states of consciousness.

Does a positive Checklist mean CHC will be awarded?

No. The guidance says a positive Checklist means the person requires a full assessment of eligibility, and it does not necessarily mean they will be found eligible. The full assessment uses the Decision Support Tool and a multidisciplinary team.

What if the scores sit just below the threshold?

A below-threshold result is a statement about the Checklist alone. The threshold is deliberately set low, the guidance allows a full assessment in occasional cases below it with a clear rationale, and you can ask the ICB to reconsider a negative outcome.
